At Gilmore Medical Practice, we invite patients with certain long-term health conditions for an annual review. These reviews help us monitor your condition, ensure your treatment is working well, and reduce the risk of future health problems.
Most patients are invited for their review during their birthday month, making it easier to remember when your annual check is due.
What happens at my appointment?
Your review will usually be carried out by one of our practice nurses.
During your appointment you may have:
- A blood test
- Your blood pressure checked
- Your height and weight measured
- A review of your medications and general health
Some patients may require additional checks depending on their medical condition.
How should I prepare for my appointment?
Please bring a urine sample, sample pots can be collected from reception before your appointment.
Please do not eat for 8 hours if possible.
Will I receive my results?
All results are reviewed by a clinician.
If your results are normal or stable and no changes are required, we will not routinely contact you but you can contact reception a week after your appointment if you wish to know your rest result
If there is a problem with your results or we need to discuss your treatment, we will contact you.
Which conditions are included?
Patients with one or more of the following conditions are usually invited for an annual review:
- Hypertension (high blood pressure)
- Stroke
- Transient Ischaemic Attack (TIA or “mini stroke”)
- Myocardial Infarction (heart attack)
- Ischaemic Heart Disease (coronary heart disease)
- Chronic Kidney Disease (CKD)
- Gout
- Fatty Liver Disease
- Peripheral Vascular Disease (Peripheral Arterial Disease)
- Intermittent Claudication
- Type 2 Diabetes
- Type 1 Diabetes
- Prediabetes
- Previous Gestational Diabetes
- Polycythaemia
- Atrial Fibrillation (AF)
- Hypercholesterolaemia (high cholesterol)
- Polycystic Ovary Syndrome (PCOS)
Asthma and COPD
Please note asthma and COPD appointments are done separately.
Other long-term conditions
Patients with other long-term conditions may also be invited for regular reviews if ongoing monitoring is recommended. If you are eligible, we will contact you when your review is due.
If you are unable to attend your appointment, please contact the practice so we can arrange another suitable time.
