The most efficient way of requesting your prescription is via the online form at the bottom of this page. Please ensure you complete ALL of the sections marked with an * otherwise the form cannot be sent.
You can also put your request in writing or use the right hand side of your previous prescription. You can deliver this in person or by post. We can only accept repeat prescription requests by phone in the most exceptional of circumstances as all requests must be in writing.
Prescriptions will be ready 5 working days after requested. Please nominate a pharmacy that you want to collect your prescription from, otherwise you can collect it from the surgery. Please take note of any messages on your prescription. We will ask you to make an appointment on a regular basis so that we can review your medication.
Please note that we are unable to fax prescriptions and can only send prescriptions to pharmacies within the local area. If you are going on holiday or if you are temporarily staying outwith our catchment area you should make arrangements to have enough medication for that period of time. If you do need medication then you should ideally register as a Temporary Resident locally or provide us with a stamped addressed envelope to send the prescription to a nearby pharmacy.
Please only order what medications are due as prescription requests will not be issued if you have ordered too early.
