How to register

To join us, first check that you live within our practice boundary.

Please note that, at certain times, we are only able to take on a limited number of patients per week depending on our current patient list size

The practice has an agreement with the NHS that we will only register patients newly moving into the practice area and who do not currently have a GP in the area or those already living in the area who do not already have a GP. If unsure please call the practice first to make sure you meet the criteria before completing any forms.

To register please print and complete BOTH forms linked below. You will need to provide proof of address and photographic identification. Please bring these forms and ID to reception between 10am-4pm Monday to Friday.

If you are going to be here for less than 3 months please DO NOT complete forms but you can contact the practice to register as a temporary patient. There are 3 practices in the building and we rotate responsibility for taking on temporary patients each week, so depending on when you enquire about this you may be asked to contact the other GP surgeries.

Temporary patients

People who are visiting a patient of our practice from another area in the UK for a period of less than 3 months, and who become unwell, are entitled to be seen as a temporary resident. Please contact the surgery for more information.

If you only require a supply of your regular medication, it may be possible to obtain this from a local pharmacy under the Unscheduled Care Scheme, or by the pharmacist liaising directly with your GP.
